| Description: | Clinica Sierra Vista is pleased to announce several full-time job openings for the Patient Navigator Program in Kern County. These positions will assist our patients with moving through the health care and social service systems in order to improve health outcomes for breast and cervical cancer, diabetes, and asthma. We are looking for highly motivated and energetic individuals who want to make a difference in the lives of our patients.

| Qualifications: | We are looking for the following skills and qualifications:
Strong interpersonal skills - ability to connect with people from diverse cultural, ethnical, and social-economic backgrounds
Aggressive problem solving skills willingness to go to great lengths to resolve a problem
Knowledge and experience in working with local community resources for both medical and non-medical services
Fluent in Spanish and English with excellent written and verbal communication skills
Possess a solid understanding of local cultural, ethnic and socio-economic conditions and barriers that impact access to health care and other services
Ability to learn basic health care information and present that to patients
Ability to work independently with little supervision and as a team player
Reliable transportation, possess a valid California drivers license, and current automobile insurance
Ability to pass a background check
Willingness to work a flexible schedule will be required to work some evenings and weekends
Prior case management experience in a clinical setting is a plus
